Dementia Beyond Disease: Enhancing Well-Being
This session will present a radically different approach to understanding and responding to distress in people who live with dementia. Using a framework of seven domains of well-being, the Guide will explain why even our best efforts at person-centered care often fall short. The guide will then outline a strength-based, proactive approach that enhances well-being, even for those with significant cognitive disability, and creates a pathway for sustained success.

Does Your Care Plan Capture the Voice of the Elders?
Care Plans currently are in the top ten deficiencies found during the annual survey process. Care Plans need to capture our elders' voices, use their strengths and needs to develop the plan and be implemented by high performing interdisciplinary teams that are focused on hearing our elders, all the while balancing regulations. This session will guide the participants in utilizing the MDS, Care Area Assessments and Elder participation to enhance the care planning process and ensure regulatory compliance is met.

Leveraging Collective Wisdom to Prepare for the Coming of QAPI Regulation
This session will make the connection between QAPI and a culture of continuous quality improvement for residents, family members and staff. QAPI is about making improvements, making residents lives better and staff lives better. Learn about practical actions that you can take to engage your residents and staff in QAPI in meaningful ways. This information is based on lessons learned from the national QAPI demonstration project and the National Nursing Home Quality Care Collaborative.

Welcome to Part 2! In this session, we'll delve into practical uses of humor through a variety of creative programs that will help you create a laughter-rich environment. Then, we'll shift gears and talk about how humor-related social interactions can be harmful, as well as helpful - and how humor can impact the mental health of elders living in community settings. You will leave this session with a tailored plan and the tools needed to create a comprehensive, personalized humor program in your environment

The Journey to "Know The Person": Exploring New Questions
Clinical care providers (Nursing, PT/OT and others), as well as chaplains, gather volumes of data about each resident. Much of the data frames "what" the patient is (widowed, school teacher, Methodist) and "how" she is (CHF, diabetes). If we knew more of "who" she is (her character, interests, even passions), would it change the way we care for the resident? This session will explore this question and provide a tool for discovery of the resident as she would want to be known.

The journey to a person-centered model of care begins with wise leadership. Formal Leaders must grow themselves so that they can grow others. Elevating the "Art" of person-centered leadership includes understanding the "Science" that drives all human behavior, including our own. Focusing on your own leadership challenges, this session will take you on an exploration of person-centered leadership that will both enlighten and inspire you.
